Background Check Authorization
By signing below I authorize Wauwatosa Crime Stoppers and its authorized representatives to conduct a background investigation, including criminal history, employment verification, and reference checks, for the purpose of evaluating my application for board service.

Permission to Share Information with the Board
By signing below I authorize Wauwatosa Crime Stoppers board members, legal counsel, investigators, and liaisons to share information gathered through this application and background review process with members of the Wauwatosa Crime Stoppers Board as necessary for consideration of my application.

Confidentiality & Non-Disclosure Agreement (NDA)
By signing below I understand that as a condition of serving on the Wauwatosa Crime Stoppers Board, I understand and agree that I may receive confidential, sensitive, or non-public information relating to Crime Stoppers operations, investigations, donors, law enforcement partnerships, applicants, and organizational matters. I agree to maintain the confidentiality of all such information and not disclose, copy, distribute, or use confidential information except as authorized by Wauwatosa Crime Stoppers in the course of fulfilling board responsibilities. I understand that this obligation survives my service on the board and that unauthorized disclosure may result in removal from the board and/or legal action.
